DISCLAIMER AND CONSENT FORM (Reiki Treatment)

Dear Client,

Reiki is a gentle, complementary energy healing technique that supports physical and emotional relaxation, recovery, and overall well-being. It is not a substitute for medical diagnosis, treatment, or medication.

When booking a treatment, I confirm the following: - I understand that Reiki is not a medical procedure and should not replace medical treatment or professional healthcare. - The Reiki practitioner does not diagnose, prescribe, or claim to heal medical conditions. - Reiki is a safe practice, but I do not expect a guaranteed result or outcome. The effects may vary individually. - I agree to inform the Reiki practitioner of any existing health conditions, medications, or relevant concerns that could influence the treatment. - I acknowledge that I receive Reiki treatment at my own responsibility.

CONSENT TO TREATMENT

- I consent to receive Reiki treatment, either in person or as a distance session. - The nature, purpose, and possible effects of the treatment have been explained to me, and I have had the opportunity to ask questions.

- I understand the treatment involves spiritual and energetic work and does not include invasive procedures. - I may withdraw my consent or discontinue the session at any time. - The Reiki practitioner agrees to maintain confidentiality and respect my privacy.
